Hugh Carroll Frazer was born in Martinsburg, West Virginia. He graduated from the United States Naval Academy in 1912, embarking on a career as a naval officer.

He received the Medal of Honor for his actions during the United States occupation of Veracruz in 1914. His leadership and bravery during this conflict marked him as a distinguished officer early in his career.

Frazer continued his service through both World War I and World War II, demonstrating long-standing dedication to the United States Navy across multiple global conflicts before retiring at the rank of Commander.
